Registering with the surgery
To register with the surgery:
- complete the NHS: register with Cardinal Medical Practice form online

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Please allow 2 working days for your registration to be processed.
If you would like to register in person, please attend any of our 3 sites to collect a registration form.
