New Patient Registration
If you would like to register at the practice, please attend any three of our sites to collect a registration form.
Alternatively, please download and print off the relevant form(s) below.
When you complete and return the form, if possible, please take to our Norwich Road site, to avoid any delays, as this is where our registration team are based.
You will be asked to bring a form of identification and proof of address and will be asked to complete a GMS1 form.
For children under the age of 16, you will need to arrange for the child to come to the surgery along with the birth certificate and or their red book when you return the registration form. For babies, this will be picked up at their 8 week check appointment, when the red book should be brought to the appointment.
Please note that for online access, this will be allowed for parents/guardians for children up to the age of 11, then access will cease on the child’s 11th birthday.
