New Patient Registrations
We are always happy to accept new patients and can register people who live within our Practice catchment area. Our Practice catchment area can be viewed here. Please call the surgery on 01473 741349 or ask reception for further advice or clarification.
You do not need proof of address or immigration status, ID or an NHS number. Patients aged (over five) will be offered an appointment for a New Patient Health Check with the Healthcare Assistant (HCA) within three months of registration.
For children under five who are also required to have an appointment, please bring in the Child Health red book to enable our administration team to retain on file the most up to date records of childhood immunisation details.
There will be three forms that we require you to complete upon registering, the GMS1 form, also the relevant patient questionnaire and the Object to Share Record, if you decide you do not want to share your information. Before reading the Object to Share Record form, please read the FAQs sheet. Please click on relevant link below, or collect from reception.
If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days. After 14 days’ you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient at your permanent practice. After three months’ you will have re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient, although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
